Why do my ankles swell in the heat in Spain?
In hot weather the blood vessels in the legs widen to lose heat, and more fluid leaks into the tissues; gravity then pulls it to the ankles and feet, especially if you sit or stand for long periods. It affects both legs, is worse in the evening and improves overnight. Cooling the legs, raising them, walking and limiting salt usually help within days. If only one leg swells, or there is pain, redness or breathlessness, it needs assessment.
Is it normal for ankles to swell after flying?
Mild swelling of both ankles after a flight of a few hours is common and comes from sitting still with the legs down. It should settle within a day or two of walking and resting with the legs up. Swelling that affects one leg, is painful, or comes with calf tenderness or breathlessness could be a deep vein thrombosis and must be assessed urgently, ideally with an ultrasound scan the same day.
Which medicines cause swollen ankles?
The commonest culprit is amlodipine and the other calcium-channel blockers used for blood pressure; ibuprofen and other anti-inflammatories, corticosteroids, some diabetes tablets such as pioglitazone, hormone treatments and some antidepressants can also cause it. Do not stop a prescribed medicine on your own: the doctor can confirm whether the drug is responsible and discuss alternatives with you or your usual prescriber.
How do I know if the swelling is my heart?
Heart failure usually causes swelling in both legs that builds up over days or weeks, together with breathlessness on exertion or when lying flat, waking at night short of breath, tiredness and weight gain. Swelling of both ankles alone, with none of those symptoms, in hot weather or after travel is rarely due to the heart. The doctor may arrange a blood test and an echocardiogram if the pattern is suspicious.
Can I prevent swollen ankles on long flights?
Choose an aisle seat if you can, walk every hour, do calf raises and ankle circles while seated, drink water, avoid alcohol and sleeping tablets, and wear below-knee graduated compression socks (15 to 30 mmHg), which reduce both swelling and the risk of a clot. If you have had a previous clot, recent surgery, cancer or are pregnant, ask the doctor before you travel: you may need specific prevention.
