PCOS Treatment in Spain: Symptoms, Tests and Weight Care

Common Symptoms

Polycystic ovary syndrome (PCOS) is one of the most common hormone conditions in women of child-bearing age. It affects about one in eight women. In May 2026 a global group agreed a new name: polyendocrine metabolic ovarian syndrome (PMOS). Most records in Spain still say PCOS or SOP. Common signs include:

  • Irregular periods, very long gaps between periods, or no periods
  • Extra hair on the face, chest or tummy
  • Acne that lasts past the teenage years
  • Thinning hair on the scalp
  • Weight gain, often around the waist, and finding weight hard to manage
  • Trouble getting pregnant
  • Low mood, anxiety, or poor sleep with loud snoring

PCOS weight worries are common, and they are not your fault. PCOS happens at every weight, including in slim women. Good care starts from what matters most to you: periods, skin, mood, weight or pregnancy. It comes without blame or weight stigma.

How We Treat It

In adults, PCOS is diagnosed when two of three features are present. These are irregular periods, signs of high male-type hormones, and polycystic ovaries on a scan or a high AMH blood test. Other causes must be ruled out first. Care can include:

  • A Weight Optimization consultation: a 30-minute video review of your weight, results and options
  • A review of your periods, skin, hair, mood, sleep and pregnancy plans
  • Blood tests to rule out other causes, such as thyroid problems or high prolactin, that you can have at a lab near you
  • A blood sugar check, as PCOS raises the risk of prediabetes and type 2 diabetes at any weight
  • A plan for food, activity and sleep you can keep up, with no harsh limits
  • Medicines that fit your goals, such as the combined pill for periods and skin. Medicines for blood sugar or weight, including GLP-1 therapy, are used only where clinically appropriate, following a genuine evaluation
  • Referral to gynaecology or a fertility team when needed
  • Follow-up appointments to check progress

No prescription is promised. Treatment and prescriptions are issued solely at the doctor's discretion based on evidence-based clinical guidelines. Any prescription is a private electronic prescription under Spanish law. GLP-1 medicines are not used in pregnancy and must be stopped before a planned pregnancy. Reliable birth control is needed while taking them. A video visit cannot replace a pelvic exam or an ultrasound scan. Under-18s with PCOS need care led by a specialist team.

Clinical Evaluation & Evidence-Based Protocol for PCOS Treatment

During a telemedicine consultation with Dr. Pedro Yelamo Vera (licensed medical practitioner No. 484814280 at the Official Medical College of Bizkaia), clinical evaluation adheres strictly to evidence-based medical (EBM) standards. The doctor conducts a thorough history, evaluating symptom duration, aggravating factors, prior treatments, and drug allergies to establish an accurate differential diagnosis.

Private Electronic Prescriptions (Receta Médica) Issued Under Spanish Law

When clinically indicated, Dr. Yelamo issues a private electronic prescription (Receta Médica Electrónica Privada) under Spanish law. The digital prescription contains a secure QR verification code accepted at all 22,000+ registered community pharmacies across Spain for immediate medication dispensing.

Itemized Invoices for Travel & Private Health Insurance Reimbursement

After your consultation, you receive an official medical report and an itemized invoice. The invoice shows Dr. Yelamo's licence details and tax ID. You can send it to your travel or private health insurer (such as Bupa, Allianz, Cigna, AXA or a US travel policy) to claim the cost back.

Preparing for Your Online Medical Consultation

Before your appointment, note when your symptoms started and any medicines you have already tried. Have your allergies and your usual prescriptions to hand. For a skin, rash or eye problem, sit in a well-lit room so the doctor can see it clearly.

Clinical Follow-Up & When In-Person Urgencias Are Necessary

Video care works well for common infections, mild sudden symptoms, skin rashes and repeat prescriptions. Some warning signs need emergency care in person: severe shortness of breath, a high fever you cannot bring down, chest pain or a stiff neck. If you have any of them, call 112 in Spain straight away.

When to Seek Immediate In-Person Care

Get urgent in-person care, or call 112 in an emergency, if you have:

  • Sudden, severe pain low in the tummy, or pain with fainting (possible ovarian problem or pregnancy outside the womb)
  • Very heavy bleeding that soaks a pad every hour, or bleeding with dizziness
  • Severe tummy pain that spreads to the back, with or without vomiting, if you take a GLP-1 medicine (possible pancreatitis or gallbladder problem)
  • Vomiting that will not stop, or signs of dehydration
  • Shaking, sweating or confusion if you take insulin or a sulfonylurea (possible low blood sugar)
  • Hair growth or a deeper voice that appears quickly over a few months (needs prompt tests to rule out other causes)
  • A positive pregnancy test while taking a weight-loss medicine (stop it and contact your doctor)

Frequently asked questions

How is PCOS diagnosed?

In adults, doctors use the Rotterdam approach in the 2023 international guideline. You need two of three features: irregular periods, signs of high male-type hormones, and polycystic ovaries on a scan or a high AMH test. Other causes, such as thyroid problems, must be ruled out. If you have both irregular periods and high hormone signs, a scan is not needed.

Is PCOS now called PMOS?

Yes. In May 2026 a global group agreed the new name polyendocrine metabolic ovarian syndrome (PMOS). It shows that this is a whole-body condition, not just cysts on the ovaries. Clinics and lab forms in Spain will still use PCOS or SOP for a while. Both names mean the same condition.

Why is PCOS linked to insulin resistance and weight?

Many people with PCOS have insulin resistance, so the body needs more insulin to control blood sugar. High insulin can push the ovaries to make more male-type hormones. This can worsen periods and skin, and raise the risk of type 2 diabetes. The guideline advises a blood sugar test at diagnosis and every one to three years.

Do I have to lose weight to treat PCOS?

No. Healthy habits can improve cycles, skin and blood sugar even if the scale barely moves. There is no single best PCOS diet. If you have had an eating disorder, tell your doctor. Care should never mean strict food rules.

Can PCOS be managed online, and what needs in-person care?

Much of it can be done by video: reviewing symptoms and past results, requesting blood tests, agreeing a plan and follow-up appointments. You need in-person care for a pelvic exam, an ultrasound scan or fertility treatment. The same goes for any urgent symptom listed above. The doctor will arrange a referral when needed.

Can I take a GLP-1 medicine if I want to get pregnant?

These medicines are not used in pregnancy. The product information says to stop them one to two months or more before trying to conceive. The exact time depends on the medicine. Plan this with your doctor. For help with ovulation, the guideline names letrozole as the first choice, guided by a gynaecology or fertility team.
