Back pain is common, painful, and disruptive, but most episodes are not caused by serious disease. The key question is whether your symptoms fit uncomplicated mechanical back pain or whether they suggest nerve compression, fracture, infection, or another urgent condition.

How We Treat It

The doctor will review pain location, onset, injury, leg symptoms, weakness, bladder or bowel symptoms, fever, cancer history, and previous episodes. Care may include:

  • Guidance on early activity and an exercise-based approach, which is favoured over routine imaging for uncomplicated pain
  • Advice against relying on paracetamol alone; short-term medication may be considered depending on the clinical picture
  • Symptom guidance and short-interval follow-up
  • Referral for imaging or in-person examination when indicated

NICE recommends exercise-based, noninvasive treatment for most uncomplicated low back pain. Online care is useful for triage and follow-up, but not for suspected cauda equina syndrome, fracture, or spinal infection.

When to Seek Immediate In-Person Care

Seek urgent medical attention if you have:

  • New bladder or bowel dysfunction
  • Saddle numbness
  • New leg weakness or progressive numbness
  • Fever with back pain
  • Back pain after major trauma
  • A history of cancer with new persistent back pain
  • Severe night pain or rapidly worsening pain
