Quick answer
Gout is arthritis caused by uric acid crystals in a joint. A flare brings sudden, severe pain, swelling and redness, most often in the big toe. Treatment works best when started early. Dr. Pedro Yelamo Vera, a GP licensed in Spain, can review a typical flare by video and plan treatment and prevention.
Common Symptoms
A gout flare often starts suddenly, sometimes at night, and is at its worst within about a day. It may cause:
- Sudden, severe pain in one joint, most often the big toe
- A joint that is swollen, red, hot and very tender
- Pain so bad that even a bed sheet on the joint hurts
- Flares in the ankle, foot, knee, wrist or fingers
- Flares after alcohol, a large meal, dehydration or an illness
- Hard lumps under the skin near joints (tophi) after years of flares
A gout flare is one of the most painful things a joint can go through, and it often strikes far from home or on a holiday. The good news is that flares settle with the right treatment, and the right long-term plan can stop them coming back.

How We Treat It
The doctor asks about the joint, how the pain started, past flares, your kidneys, your other conditions and every medicine you take. Care may include:
- A treatment plan for the flare, chosen to suit your health and other medicines
- Advice on rest, raising the joint and cold packs
- A review of triggers: alcohol, sugary drinks, large meals, dehydration and some medicines
- A request for blood tests for uric acid and kidney function
- Advice on long-term uric acid-lowering treatment if flares keep coming back
- A prescription when clinically appropriate, after a genuine evaluation
A first flare, a flare in an unusual joint or a joint that is hot with fever may need an in-person exam, and sometimes fluid from the joint, to rule out infection. Prescriptions are issued solely at the doctor's discretion. This service is for urgent, non-emergency care.
What causes gout?
Gout happens when there is too much uric acid in the blood. The extra uric acid forms sharp crystals in a joint, and the body reacts with sudden inflammation.
High uric acid runs in families. It is also more likely with excess weight, high blood pressure, kidney disease, a lot of alcohol (beer and spirits most of all), sugary drinks and some medicines, such as water tablets.
How is a gout flare treated?
A flare is treated with medicine that calms the inflammation, started as early as possible. The right choice depends on your kidneys, stomach, heart and the other medicines you take, so it needs a doctor's review.
At home, rest the joint, raise it and use a cold pack wrapped in a towel for up to 20 minutes at a time. Drink plenty of water. Most flares settle within one to two weeks.
When do you need treatment to lower uric acid?
Long-term treatment to lower uric acid is offered when flares keep coming back, or when there are tophi, kidney stones, joint damage or kidney disease. It is taken every day, even when you feel well.
Uric acid is best measured a few weeks after a flare has settled, because it can read low during one. If you already take uric acid-lowering treatment, do not stop it during a flare unless your doctor tells you to.
Can I see a doctor for gout in English in Spain?
Yes. Dr. Pedro Yelamo Vera holds secure video consultations in English or Spanish every day from 07:30 to 20:30, Spain time. He reviews the flare, plans treatment and prevention, and refers you for an in-person check when a video call is not enough.

Preparing for Your Online Medical Consultation
Before your appointment, note when your symptoms started and any medicines you have already tried. Have your allergies and your usual prescriptions to hand. For a skin, rash or eye problem, sit in a well-lit room so the doctor can see it clearly.
Clinical Follow-Up & When In-Person Urgencias Are Necessary
Video care works well for common infections, mild sudden symptoms, skin rashes and repeat prescriptions. Some warning signs need emergency care in person: severe shortness of breath, a high fever you cannot bring down, chest pain or a stiff neck. If you have any of them, call 112 in Spain straight away.
When to Seek Immediate In-Person Care
Get urgent in-person care, or call 112 in an emergency, if a painful joint comes with:
- Fever, shivering or feeling very unwell
- Pain and swelling that keep getting worse
- Redness spreading up the leg or arm
- Several joints hot and swollen at the same time
- A joint replacement in the painful joint
- A weak immune system, for example during cancer treatment
Frequently asked questions
Can gout be treated online in Spain?
Often, yes, when you have had gout before and the flare looks typical. A first flare, a hot joint with fever or a flare in an unusual joint may need an in-person exam first.
How long does a gout flare last?
Pain is usually worst within the first day. With early treatment most flares settle within a few days to two weeks.
What foods and drinks trigger gout?
Beer and spirits, sugary drinks, red meat, offal and some seafood can raise uric acid. Dehydration and very large meals can also set off a flare. Diet helps, but many people also need medicine.
Is gout the same as high uric acid?
No. Many people have high uric acid and never get gout. Gout is the joint inflammation caused when uric acid forms crystals in a joint.
Can I get my gout medication in a Spanish pharmacy?
Gout medicines need a prescription in Spain. After a video review, the doctor can issue a Spanish e-prescription when it is clinically appropriate. Neither issuance nor acceptance is guaranteed.
