Mounjaro is tirzepatide, a prescription-only injection. In Spain it is licensed for type 2 diabetes and for weight management in adults with obesity, or with overweight and a related health problem. It acts on two gut hormones, GIP and GLP-1. Spain's public health system does not fund it, so you pay the full price.

In short
  • It is injected under the skin with a prefilled pen, alongside changes to diet and activity.
  • It is sold only in pharmacies, with a prescription, and is not publicly funded.
  • Its most common side effects affect the gut.

What is Mounjaro?

Mounjaro is the brand name of tirzepatide. It acts on two gut hormones at once: GIP and GLP-1. It comes in pens that you inject under the skin of the belly, thigh or upper arm. There is no tablet form. Your doctor always decides the plan.

Is Mounjaro the same as Zepbound?

Yes, it is the same medicine. In the US, tirzepatide is sold as Mounjaro for diabetes and as Zepbound for weight loss. In Spain, both uses come under one name, Mounjaro. A US prescription is not valid in a Spanish pharmacy, as our guide on using a foreign prescription in Spain explains.

What is Mounjaro used for in Spain?

Its EU product information, valid in Spain, lists two uses:

  • Type 2 diabetes that diet and exercise do not control well, alone or with other diabetes medicines. It is also licensed from age 10, under a children's specialist team.
  • Weight management in adults with a body mass index (BMI) of 30 or more, or 27 to under 30 with at least one weight-related health problem. Examples are prediabetes, high blood pressure, high cholesterol, sleep apnoea and heart disease.

In both cases it is used with a lower-calorie diet and more physical activity. See how GLP-1 weight-loss medicines work.

How does Mounjaro work?

Tirzepatide copies two hormones your gut releases when you eat, GIP and GLP-1, but lasts much longer. It helps the pancreas release insulin when blood sugar rises. It also cuts appetite and makes the stomach empty more slowly. So you eat less and your blood sugar falls.

Do you need a prescription for Mounjaro in Spain?

Yes. Mounjaro is a prescription-only medicine in Spain. A pharmacy can only hand it over with a valid prescription, and it is never sold online. First, a doctor checks your health and your other medicines.

Is Mounjaro covered by Spain's public health system?

No. In December 2025 the Ministry of Health's pricing committee kept its decision not to fund Mounjaro, and we have found no change since. People who use it pay for it themselves. This can change.

How much does Mounjaro cost in Spain?

The price depends on which pen your doctor prescribes and on the pharmacy. As it is not funded, you pay the full price. Ask your pharmacy. Be wary of any website selling it cheap or without a prescription.

What are the side effects of Mounjaro?

The most common are gut problems: nausea, diarrhoea, vomiting, constipation and tummy pain. They are usually mild and tend to appear at the start or when the plan is stepped up. Wind, heartburn, tiredness and skin redness are also common.

Less common but serious: pancreatitis, gallstones and dehydration, which can harm the kidneys. With insulin or a sulfonylurea, low blood sugar is more likely. More in our guide to weight-loss injection side effects.

What happens when you stop Mounjaro?

Most people regain a good part of the weight they lost. In the SURMOUNT-4 trial, people switched to a dummy injection regained much of it; those who stayed on tirzepatide did not. Plan any change with your doctor. See stopping weight-loss medication and weight regain.

Who should not use Mounjaro, or needs extra care?

  • People allergic to tirzepatide or any of its ingredients.
  • Pregnancy: it is not advised. Use birth control, and talk to your doctor before you try for a baby.
  • Breastfeeding: ask your doctor.
  • Past pancreatitis, severe gut disease or diabetic eye disease: extra care is needed.
  • Medullary thyroid cancer or MEN 2, in you or your family: it caused thyroid tumours in rats. Tell your doctor.
  • Surgery with an anaesthetic or sedation: tell the team.

Mounjaro vs Ozempic and Wegovy: what is the difference?

  • Active ingredient: Mounjaro is tirzepatide. Ozempic and Wegovy are semaglutide.
  • How they work: tirzepatide acts on GIP and GLP-1. Semaglutide acts on GLP-1 only.
  • Licensed use: Mounjaro for diabetes and weight, Ozempic for diabetes, Wegovy for weight.
  • Public funding: Spain funds semaglutide for some adults with type 2 diabetes, under set rules. Mounjaro is not funded.

In the SURMOUNT-5 trial, adults with obesity but no diabetes lost more weight and more waist on average with tirzepatide than with semaglutide. An average does not predict your result. Your doctor chooses, and you should not swap on your own.

Fake pens and buying online

Spain's medicines agency (AEMPS) and the EMA have warned about illegal tirzepatide and other GLP-1 products sold online and on social media. They may hold none of the real drug, or something harmful. In Spain, prescription medicines cannot be sold online. More in our guide to counterfeit weight-loss pens in Spain.

When to see a doctor

  • Vomiting or diarrhoea that will not settle, or stops you drinking.
  • A lump in the neck, a hoarse voice or trouble swallowing.
  • Pain in the upper right side of the tummy, fever or yellow skin.

When to call 112

  • Severe, constant tummy pain that may spread to the back.
  • Swelling of the face, lips or throat, or difficulty breathing.
  • Low blood sugar with confusion, fainting or a fit.
