Ozempic is an injection of semaglutide, given with a prefilled pen. In Spain it is licensed for adults with type 2 diabetes that is not well controlled by diet and exercise. It is not licensed for weight loss: the semaglutide product licensed for weight management is Wegovy. Ozempic always needs a prescription in Spain.

In short
  • It is not licensed for weight loss. Wegovy is.
  • It is prescription-only, and in Spain it cannot be sold online.
  • Fake pens are a real risk: only get it from a pharmacy.

What is Ozempic?

Ozempic is a brand of semaglutide, a GLP-1 medicine. It copies a gut hormone the body releases when you eat. It helps the pancreas release insulin when blood sugar is high. It also slows down the stomach and lowers appetite. The European Medicines Agency (EMA) approved it in 2018. It is the same medicine sold as Ozempic in the US and the UK. Read more on how GLP-1 medicines work.

What is Ozempic used for?

It treats adults with type 2 diabetes that is not well controlled, along with diet and exercise. It can be used on its own when metformin is not suitable, or with other diabetes medicines, including insulin. The leaflet says it may also help prevent heart disease and slow the loss of kidney function.

Can you use Ozempic for weight loss in Spain?

It is not licensed for that. Many people with diabetes lose weight on Ozempic, but its licence is for type 2 diabetes. Using it only to lose weight is off-label use. During the shortage, the Spanish medicines agency (AEMPS) asked doctors to keep it for its licensed uses.

The semaglutide licensed for weight management is sold under another name: Wegovy. It is for adults with obesity, or who are overweight with a weight-related health problem, along with diet and exercise. If you want help with your weight, start with a medical weight review, not with a brand name.

Do you need a prescription for Ozempic in Spain?

Yes. In CIMA, the AEMPS medicines database, Ozempic is listed as prescription-only. You cannot buy it over the counter. Spain also bans the online sale of prescription medicines. A website that sells Ozempic without a prescription is breaking the law.

Is Ozempic covered by the Spanish public health system?

The public health system (SNS) funds Ozempic for type 2 diabetes, under conditions set by the Ministry of Health. It does not fund it for weight loss. Medicines licensed for weight management, such as Wegovy, are not publicly funded at present. These rules can change, so ask your doctor or pharmacist.

How much does Ozempic cost in Spain?

The price depends on the pen your doctor prescribes and on whether you have public funding. With a public prescription you pay only your share. With a private one you pay the full price. Your pharmacy can tell you the exact amount. Be wary of cheap offers outside a pharmacy.

Ozempic side effects

The most common are nausea and diarrhoea. They are usually mild and ease over time. Other common ones:

  • vomiting, tummy pain, constipation, heartburn and wind;
  • dizziness, headache and tiredness;
  • gallstones;
  • worse diabetic eye disease (retinopathy);
  • low blood sugar when used with insulin or sulfonylureas.

Uncommon or rare but serious: inflamed pancreas (pancreatitis), a blocked bowel and allergic reactions. The EMA has also confirmed a very rare eye nerve problem that causes sudden loss of vision. Vomiting and diarrhoea can make you dehydrated and, in rare cases, harm the kidneys. See our guide to weight-loss injection side effects.

What happens when you stop Ozempic?

If you have diabetes, your blood sugar may rise. The leaflet says not to stop without talking to your doctor. Weight also tends to come back. In weight trials of semaglutide, people who stopped regained much of the weight they had lost. Read more in stopping weight-loss medication.

Who should not use Ozempic?

  • People allergic to semaglutide or any of its ingredients.
  • People with type 1 diabetes or ketoacidosis: Ozempic does not replace insulin.
  • Women who are pregnant or breastfeeding. If you plan a pregnancy, the product information says to stop it at least two months before.

Talk to a doctor first if…

  • you have had pancreatitis;
  • you have diabetic eye disease or other eye problems;
  • your stomach empties very slowly (gastroparesis), or you have kidney disease;
  • you or a close relative have had medullary thyroid cancer or MEN2;
  • you are due to have surgery or deep sedation.

Fake Ozempic pens

The EMA has warned about fake Ozempic pens. The AEMPS warns of a rise in illegal products sold as GLP-1 medicines. A fake pen may hold no medicine, or the wrong one. Only get Ozempic from a pharmacy, with a prescription. Never share your pen or needles. Read our guide to counterfeit weight-loss pens in Spain.

When to see a doctor

  • Your vision suddenly drops.
  • You notice a lump in your neck, a hoarse voice or trouble swallowing.
  • Vomiting or diarrhoea does not stop and you cannot keep fluids down.

When to call 112

  • Severe, lasting tummy pain that may spread to your back.
  • Trouble breathing, or swelling of the face, lips, tongue or throat.
  • Confusion, cold sweats or passing out from low blood sugar.
  • A swollen, painful tummy with vomiting and no stools or wind.

Frequently asked questions about Ozempic

Can I buy Ozempic over the counter in Spain?

No. It is prescription-only, and pens sold without a prescription may be fake.

Is Ozempic licensed for weight loss?

No. Its licence is for type 2 diabetes. The semaglutide licensed for weight management is Wegovy, and a doctor decides if it suits you.

Can I use my UK or US Ozempic prescription in Spain?

Not reliably. A UK paper prescription is accepted only if the pharmacist agrees, and a US one is not valid. See using a UK prescription in Spain.

Is there an Ozempic pill?

Ozempic is only an injection. Semaglutide tablets for type 2 diabetes are sold as Rybelsus, which also needs a prescription.

What is "Ozempic face"?

It is not a medical term or a listed side effect. It describes how the face can look after a lot of weight loss.

Can Ozempic cause blindness?

The EMA lists an eye nerve problem as a very rare side effect. If your vision suddenly drops, get your eyes checked urgently.
