Erectile dysfunction (ED) — difficulty getting or keeping an erection firm enough for sex — is one of the most common reasons men see a doctor and one of the least talked about. If you are an expat, digital nomad or visitor in Spain, you can discuss it in 2026 by video with a licensed, English-speaking GP, without a waiting room and without navigating the conversation in a second language.

TL;DR
  • ED is common and usually has a treatable cause: circulation, stress, medication, sleep or alcohol.
  • Sildenafil and similar ED medicines are prescription-only in Spain and most of the EEA.
  • An online GP can assess you and, where clinically appropriate following a genuine evaluation, issue an EU e-prescription.
  • An erection lasting more than 4 hours, chest pain, or sudden vision or hearing loss needs urgent care — not a routine video call.
  • No prescription is ever guaranteed; the medical assessment decides.

Why this matters

Most men experience ED at least occasionally, and for many it becomes persistent enough to affect confidence, relationships and sleep. It is also very often an early clue about general health: the small blood vessels that fail during an erection are the same ones affected by high blood pressure, high cholesterol, smoking and diabetes. A proper consultation looks at the whole health picture, not just the symptom — and that is exactly what turns an embarrassing topic into a manageable one.

For men living abroad there is an extra layer. You may not have a regular doctor yet, you may not know what a Spanish pharmacy can and cannot sell, and explaining an intimate symptom in Spanish is a real barrier. None of that changes the medicine — but it changes how easy it is to start.

The struggle: asking for help in a foreign health system

Picture the usual route. You walk into a pharmacy, stand in a queue, and try to ask for sildenafil in a language you are not fluent in. The pharmacist explains — correctly — that in Spain it is prescription-only. You leave with nothing, and the problem stays unsolved for another month.

Or you consider the public system and stop at the first obstacle: registering, waiting for an appointment, and then describing the most private symptom you have to a doctor you have never met, in hurried Spanish.

This is a common and understandable position to be in. ED is a medical topic like any other, and there is a calm, private way to deal with it.

What actually helps

ED usually has more than one contributing factor, and several of them are modifiable:

  • Circulation and metabolic health — blood pressure, cholesterol, blood sugar and smoking status all matter, because erections depend on blood flow.
  • Medication side effects — some blood pressure medicines, antidepressants and other common drugs can contribute. Changing them is a doctor's decision, never your own.
  • Alcohol and sleep — heavy drinking and poor sleep both measurably worsen erectile function.
  • Stress and anxiety — especially performance anxiety, which is common in younger men and responds well to honest discussion.
  • Hormones — low testosterone is worth ruling out with a simple blood test in persistent cases.

The first step is not a tablet. It is an honest assessment of which of these apply to you.

How an online GP consultation works

  1. Book a video slot. Appointments are typically available within 24 hours. Choose a private room and use headphones — the call is one-to-one and confidential.
  2. History and medication review. Have a list of what you take, including supplements. Be specific about how long the difficulty has lasted and whether morning erections are still normal — that detail genuinely changes the assessment.
  3. Options discussion. For many men, tablets known as PDE5 inhibitors (sildenafil, tadalafil and similar) are a reasonable first-line option — but only where clinically appropriate, following a genuine evaluation. They are unsafe with certain medicines, particularly nitrates for chest pain, so the review matters.
  4. E-prescription. If a prescription medicine is appropriate and safe for you, the doctor can issue an official Spanish e-prescription, valid across the EEA and redeemable at Spanish pharmacies. This is never automatic — it follows the assessment.
  5. Underlying-cause checks. The GP can request blood tests (glucose, cholesterol, testosterone), review your blood pressure, and arrange follow-up. Treating a cause beats masking a symptom.

What a video consultation cannot do

  • It cannot replace a physical examination when one is genuinely needed.
  • It cannot guarantee a prescription, a specific medicine, or an outcome — every decision follows the clinical evaluation.
  • It does not cover controlled substances.
  • If the assessment suggests a problem better handled in person, a good outcome is a referral, not a tablet.

When ED needs urgent care

A few situations are emergencies and should never wait for a routine video call:

  • An erection lasting more than 4 hours (priapism) — go to an emergency department; untreated it can cause lasting damage.
  • Chest pain or severe breathlessness during sex or after taking an ED tablet — call 112 in Spain immediately.
  • Sudden loss of vision or hearing after taking an ED medicine — stop the medicine and seek emergency care at once.

Everything else can start with a scheduled consultation.

Frequently Asked Questions

Can I buy sildenafil over the counter in Spain?

No. Sildenafil and all similar ED medicines are prescription-only in Spanish pharmacies. A pharmacy cannot sell them without a valid prescription.

Can an online doctor prescribe ED medication in Spain?

A licensed doctor can prescribe it where clinically appropriate, following a genuine evaluation. The video assessment reviews your medical history and current medicines first, because these medicines are unsafe for some men.

How quickly can I get an appointment?

Appointments and responses are typically available within 24 hours, and the prescription, if one is issued, arrives on your phone as an EU e-prescription.

Is a Spanish e-prescription valid in other EU countries?

Prescriptions issued in Spain are valid across the European Economic Area under cross-border healthcare directives, though whether a specific pharmacy abroad can dispense it depends on that country's system.

What if the tablet does not work?

That is a normal reason for a follow-up. The dose or the medicine can be reviewed, and persistent failure usually prompts a closer look at underlying causes.

Do I need blood tests?

For persistent ED they are often recommended, to check blood sugar, cholesterol and testosterone. The GP can request them and review the results with you.

One last thing

In younger men, ED is frequently driven by anxiety, alcohol or sleep rather than blood vessels — and it still deserves one honest consultation, because ruling out a physical cause takes minutes and removes months of quiet worry. Verdict: book the assessment before buying anything from the internet; unregulated ED tablets are one of the most counterfeited medicines in the world.
